Connecting for Care: The Congregational Care Network
Being discharged from the hospital to an empty home can diminish health outcomes. A philanthropy-fueled program pairs patients with volunteers from faith communities to help reduce re-admissions and emergency visits and improve patients’ overall well-being. The Congregational Care Network understands the reach and impact of faith communities in their neighborhoods and trains “companions” to connect with IU Health patients who need extra support after being discharged.
